Posted: Aug 24, 2026 Patient Advocate (Bilingual) Westchester Community Health Center - Mount Vernon, NY business_center Full-time Salary: $40,000.00 - $44,290.00 Annually Application Deadline: Jun 30, 2026 Nonprofit Job Summary The Patient Advocate will provide direct supportive services to the Mount Vernon Neighborhood Health Center network Inc., d/b/a Westchester Community Health Center, patients, and their families. Advocates will promote patients with the provision of culturally sensitive, high quality comprehensive health care. Primary Responsibility Of The Position Facilitate access to are and help patients navigate the health care system. Assist the patients by providing them with translations ad culturally sensitive services to clarify diagnosis and treatment instructions. Work collaboratively with medical providers and clinical/nonclinical staff from all departments to remove any barrier that may prevent patients from getting the best from their visits. Service arrangement for medical transportation. Referral linkage. Assist patients to locate outside specialists and services and help them book appointments. Connect patients to outside resources and services available. Participate in health advocacy events to raise awareness about preventive health and access to care. Provide education on preventative health and management of diseases. Document every patient’s encounter and address complaints. Make arrangements for sign language interpreters for patients requiring the services. Performs other related duties as assigned.
