About the role
We are seeking a registered nurse to provide comprehensive home health care, focusing on patient-centered, individualized treatment.
Responsibilities
- Serve as the primary clinical partner for patients and families, delivering personalized in-home nursing care to promote recovery, safety, and independence.
- Conduct thorough assessments to develop tailored care plans addressing physical, emotional, and environmental needs.
- Provide skilled interventions such as wound care, infusions, medication management, and chronic disease care.
- Educate and empower patients and caregivers with clear instructions and support for symptom and medication management.
- Monitor patient progress, recognize early changes, and collaborate with physicians and care teams to adjust plans and prevent hospitalizations.
- Ensure care continuity and high-quality outcomes through effective coordination and documentation.
Requirements
- Active RN license in the practice state
- At least one year of clinical nursing experience
- Patient-focused, organized, and motivated to deliver quality care
- Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain)
