Part-Time Home Health Nurse

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases, often post-hospital discharge, under RN supervision
• Collaborate with physicians, RNs, CNAs, and aides to ensure quality patient care
• Perform patient vitals, assessments, document progress, and communicate changes to supervisors
• Assist RNs in implementing care plans and educating patients and families on self-care, medication, and home safety
• Coordinate with the interdisciplinary team for patient care and discharge planning

Requirements

• Active, unrestricted LPN license in the designated state
• Minimum of one year of clinical LPN experience
• Current CPR certification
• Reliable transportation, valid driver’s license, and current insurance
