Part Time Home Health Registered Nurse

About the Role

This is a part-time home health registered nurse position, primarily serving patients in Brevard County on weekends. The role involves providing one-on-one, individualized nursing care in patients' homes, focusing on recovery, safety, and independence.

Responsibilities

• Conduct comprehensive in-home assessments and develop patient-centered care plans.
• Deliver skilled interventions such as wound care, infusions, medication management, and chronic disease management.
• Educate and empower patients and caregivers for symptom and medication management.
• Monitor patient progress, communicate with healthcare teams, and adjust care plans accordingly.
• Ensure accurate documentation to support quality and compliance.
• Coordinate care across providers to ensure continuity and high-quality outcomes.

Requirements

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
