Key Responsibilities
- Providing individualized, one-on-one nursing care in patients' homes to promote recovery, safety, and independence.
- Performing comprehensive in-home assessments and developing patient-centered care plans.
- Delivering skilled interventions such as wound care, infusions, medication management, and chronic disease care.
- Educating and empowering patients and caregivers on symptom management and daily care.
- Monitoring patient progress, identifying changes, and communicating with physicians and care teams to adjust plans as needed.
- Ensuring care coordination and accurate documentation to support quality and compliance.
Qualifications And Skills
- Active RN license in the state of practice.
- Minimum of one year of clinical nursing experience.
- Positive attitude, patient-focused, organized, and motivated to deliver high-quality care.
- Valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
