Part Time Home Health Registered Nurse

Role Overview

This position is for a part-time Registered Nurse (RN) providing home health care within a designated service area. The nurse serves as the primary clinical partner for patients and families, delivering personalized, one-on-one nursing care to promote recovery, safety, and independence.

Key Responsibilities

• Conduct comprehensive in-home assessments and develop patient-centered care plans.
• Perform skilled interventions such as wound care, infusions, catheter care, medication management, and chronic disease management.
• Educate and empower patients and caregivers with clear instructions and support.
• Monitor patient progress, identify early changes, and communicate effectively with interdisciplinary teams to adjust care plans.
• Cook up care across providers to ensure continuity and high-quality outcomes.
• Complete accurate documentation supporting quality and compliance.

Qualifications & Skills

• Active RN license in the practice state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Reliable transportation, valid driver’s license, and CPR certification (or ability to obtain).
