Part Time Home Health Registered Nurse

Role: The Registered Nurse provides skilled nursing care in clients' homes, following care plans and physicians' orders, while maintaining confidentiality and professional boundaries. Responsibilities Develop individualized care plans with measurable goals based on assessments, physician input, and client needs.Educate clients and families to support health and wellness.Orient and train primary caregivers to optimize client functioning.Coordinate ongoing care through documentation and communication with physicians and caregivers.Deliver in-home care using skills such as patient assessment, wound care, infusions, catheter care, and PICC line dressing changes. Qualifications & Skills Active RN license in the applicable state.Valid CPR/BLS certification and driver’s license.Preferred: Medicare skilled nursing experience, basic Oasis understanding, and at least 1 year of RN experience.
