About the Role

This position involves providing personalized, in-home nursing care to promote patient recovery, safety, and independence. The nurse conducts comprehensive assessments, implements skilled interventions such as wound care, infusions, medication management, and chronic disease control, and educates patients and caregivers for self-management. They monitor progress, communicate with healthcare teams, coordinate care, and ensure accurate documentation to support quality and compliance.

Responsibilities

  • Serve as primary clinical partner for patients and families
  • Perform in-home assessments and develop patient-centered care plans
  • Deliver skilled nursing interventions tailored to individual needs
  • Educate and empower patients and caregivers
  • Monitor patient progress and communicate with interdisciplinary teams
  • Ensure care continuity and complete accurate documentation

Qualifications & Skills

  • Active RN license in the practice state
  • Minimum of one year of clinical nursing experience
  • Patient-focused, organized, and motivated by delivering high-quality care
  • Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain)
