About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct comprehensive in-home health assessments for Medicare Advantage and other plan members. Your primary duties include reviewing medical history, performing physical exams, screenings, and point-of-care testing, identifying diagnoses, and documenting findings for care management. You will communicate with primary care providers, recognize urgent situations, educate members on health, medications, and social determinants, and provide referrals as needed.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members
• Review medical history and perform physical exams, screenings, and point-of-care testing
• Identify diagnoses and document findings for care management
• Communicate with primary care providers
• Recognize and respond to urgent medical situations
• Educate members on health, medications, and social determinants of health
• Provide referrals as needed

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, or NCCPA)
• Reliable transportation and valid driver’s license
• Ability to navigate home environments and perform assessments

Preferred Qualifications

• 1+ years of clinical experience
• Proficiency with electronic medical records
• Excellent communication skills, particularly with geriatric and Medicare populations
