About the role
Join a patient-centered, field-based role as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducting in-home health assessments for Medicare members.
Responsibilities
- Performing comprehensive assessments, including medical history, vital signs, physical exams, and screenings
- Documenting diagnoses for care management and collaborating with primary care providers
- Recognizing urgent situations and intervening appropriately
- Educating patients on health, medications, and adherence
- Addressing social determinants of health and providing referrals
Requirements
- Active, unrestricted licensure in the practice state
- For NPs: Certification from ANCC or AANP; for PAs: NCCPA certification and state licensure
- Reliable transportation and ability to navigate varied home environments
- Ability to work independently within scope of practice
Preferred Skills
- 1+ years of clinical experience, especially in geriatrics or home health
- Proficiency with electronic medical records and technology
- Strong communication skills with older or Medicare populations
- Ability to transport equipment and navigate stairs during visits
