About the role

We are seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare and plan members. This non-prescribing, field-based role focuses on improving health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Perform comprehensive in-home health assessments
• Document diagnoses and communicate findings to primary care providers
• Recognize and respond to urgent health situations
• Educate members on health management and preventive care
• Address social determinants of health

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant licensure
• Relevant certification (e.g., ANCC, AANP, NCCPA)
• Reliable transportation and ability to navigate home environments

Preferred Qualifications

• Clinical experience in primary care, geriatrics, or Medicare populations
• Proficiency with Electronic Medical Records (EMR)
• Strong communication skills
