About the Role

This position involves conducting comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care, health education, and care gap closure. The role emphasizes collaboration with primary care providers, addressing social determinants of health, and recognizing urgent situations.

Responsibilities

• Performing medical history reviews, physical exams, vital signs, and screenings
• Documenting diagnoses and communicating findings to primary care providers
• Educating members on health management and medication adherence
• Addressing social factors impacting health and providing referrals
• Recognizing and responding to urgent health issues

Requirements

• Active licensure and certification (NPs: ANCC/AANP; PAs: NCCPA)
• Unrestricted driver’s license and reliable transportation
• Ability to navigate diverse home environments and perform physical assessments

Preferred Qualifications

• Clinical experience in family, geriatric, or home health settings
• Proficiency with EMR systems
• Strong communication skills
