About the role

Home-based, patient-focused role with flexible scheduling, no on-call, weekends, or holidays.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, physical exams, vital signs, and screenings
• Document diagnoses for care management and collaborate with primary care providers
• Identify urgent situations and intervene appropriately
• Educate patients on disease management, medications, and adherence
• Address social determinants of health and provide referrals as needed

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, NCCPA) in relevant fields
• Valid driver’s license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• For states allowing independent practice, eligibility approval from licensing boards

Preferred Skills

• 1+ years of clinical experience, especially in geriatrics or home health
• Proficiency with electronic medical records and technology
• Physical ability to transport equipment and navigate stairs
• Strong communication skills with Medicare populations
