About the role
We are seeking a Nurse Practitioner or Physician Assistant to join a well-established adult primary care practice within a collaborative team.
Responsibilities
- Provide comprehensive primary care for adults
- Conduct wellness visits and screenings
- Diagnose and treat acute and chronic conditions such as hypertension, diabetes, and respiratory illnesses
- Manage lab and diagnostic results
- Develop personalized care plans
- Educate patients on medications and lifestyle changes
- Utilize telemedicine and patient portals to improve engagement
Requirements
- Current New York State licensure
- Graduation from an accredited NP or PA program
- DEA registration or eligibility
- Strong clinical and communication skills
Preferred Qualifications
- Experience in Internal Medicine, Primary Care, or Urgent Care
- Experience managing chronic disease populations

Pay
