Nurse Practitioner/Physician Assistant

About the role

Join a dedicated adult primary care team within an established practice as a Nurse Practitioner or Physician Assistant.

Responsibilities

• Provide comprehensive primary care for patients aged 18 and older
• Conduct wellness visits and preventive screenings
• Diagnose and treat acute and chronic conditions such as hypertension, diabetes, COPD, and obesity
• Interpret diagnostic results and develop personalized treatment plans
• Educate patients on medications and lifestyle changes
• Collaborate with a multidisciplinary team
• Utilize telemedicine and patient portals to improve access and engagement
• Maintain accurate electronic medical records
• Coordinate care within an integrated healthcare setting that offers on-site labs, imaging, and specialty services

Requirements

• Current NY licensure as a Nurse Practitioner or Physician Assistant
• Graduate from an accredited program
• DEA eligibility
• Strong clinical decision-making and excellent communication skills

Qualifications

Prior experience in Internal Medicine, primary care, or chronic disease management is preferred, but new graduates with interest in adult primary care are encouraged to apply.

Schedule

Full-time outpatient role with a Monday–Friday schedule.
