About the role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct in-home health assessments for Medicare Advantage members to improve health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Perform comprehensive assessments, including history review, vital signs, physical exams, and screenings
• Document diagnoses and communicate findings to PCPs
• Identify urgent/emergent issues and intervene when necessary
• Educate patients on disease management and medication adherence
• Address social determinants of health and provide referrals

Qualifications and Skills

• Active, unrestricted licensure in practicing state and relevant certifications (ANCC, AANP, NCCPA)
• Reliable transportation and ability to navigate home environments
• Proficiency with electronic medical records and technology
• Preferred: 1+ years clinical experience, experience with geriatric or Medicare populations
