About the Role

Perform comprehensive in-home health assessments for Medicare Advantage members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers. The role involves conducting physical exams, reviewing medical history, medication reconciliation, screenings, and identifying urgent issues.

Responsibilities

• Perform in-home assessments, including vital signs and screenings
• Document diagnoses and assist in care management planning
• Collaborate with primary care providers (PCPs) and recognize urgent/emergent situations
• Educate patients on health conditions and medications
• Address social determinants of health and provide referrals

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, NCCPA) in relevant fields
• Reliable transportation and ability to navigate home environments
• Strong communication skills, with experience working with geriatric or Medicare populations preferred
• Proficiency with electronic medical records and technology
• Ability to lift equipment up to 30 pounds and navigate stairs

Pay

Salary ranges from $125,000 to $175,000, including base pay and bonuses.
