About the Role

Responsibilities
- Conducting comprehensive assessments such as health history, medication reconciliation, vital signs, and physical exams
- Identifying diagnoses and documenting care needs
- Communicating findings to primary care providers to address care gaps
- Recognizing urgent situations and intervening appropriately
- Educating patients on disease management and medication adherence
- Addressing social determinants of health and providing referrals
Qualifications
- Active licensure as a Nurse Practitioner or Physician Assistant in the practicing state
- Unrestricted driver's license and reliable transportation for home visits
- Ability to navigate varied home environments and perform assessments in different settings
- For NPs: Certification from ANCC or AANP; PAs: NCCPA certification and state licensure
- Preferred: 1+ years of clinical experience, proficiency with electronic medical records, and strong communication skills with geriatric populations
