About the role

Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage members across Iowa.

Responsibilities

• Perform comprehensive assessments: medical history, physical exam, vital signs, screenings, and point-of-care tests.
• Identify diagnoses, document findings, and communicate with primary care providers.
• Recognize urgent situations and provide education on health management and social determinants.

Qualifications

• Active, unrestricted licensure in practicing state.
• Active ANCC or AANP certification if an NP.
• Valid driver's license and reliable transportation for home visits.
• Ability to navigate diverse home environments and perform physical assessments.

Preferred Skills

• 1+ years of clinical experience, preferably in geriatrics or home health.
• Proficiency with electronic medical records and technology.
• Strong communication skills with Medicare populations.
