Nurse Practitioner - House Calls Traveler

Responsibilities

• Conducting comprehensive in-home health assessments, including medical history, physical exams, screenings, and point-of-care tests
• Identifying diagnoses and documenting findings for care management
• Communicating with primary care providers to address care gaps
• Recognizing urgent situations and intervening appropriately
• Educating patients on health conditions, medications, and adherence
• Addressing social determinants of health and providing referrals as needed

Qualifications

• Active licensure and certification (ANCC/AANP for NPs), with ability to practice independently if applicable
• Unrestricted driver's license and reliable transportation
• Ability to navigate various home environments and perform assessments physically
• Preferred: 1+ years clinical experience, proficiency with electronic records, and strong communication skills with older or Medicare populations
