Nurse Practitioner - House Calls - Statewide Traveler

Responsibilities

• Conducting comprehensive in-home health assessments, including medical history, vital signs, physical exams, and screenings.
• Documenting diagnoses for care management and communicating findings to primary care providers.
• Identifying urgent issues, providing patient education, and addressing social determinants of health with referrals.

Qualifications

• Active, unrestricted licensure and certification (ANCC or AANP in relevant specialties).
• Valid driver's license, reliable transportation, and ability to navigate varied home environments.
• Ability to perform assessments in different home settings, including sitting, standing, and kneeling.

Preferred Skills

• At least one year of clinical experience, especially in family, geriatric, or home health care.
• Proficiency with electronic medical records and strong communication skills with Medicare populations.
• Ability to lift equipment up to 30 pounds and navigate stairs.
