Role Overview

Seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members across Idaho. This field-based role focuses on preventive care, health education, and care gap closure, collaborating with primary care providers to improve patient outcomes.

Key Responsibilities

• Conduct comprehensive assessments including history, medication reconciliation, vital signs, and physical exams
• Identify diagnoses and document findings for care management
• Communicate with PCPs to address care gaps
• Recognize urgent situations and provide appropriate interventions
• Educate members on disease management, medications, and social determinants of health

Qualifications & Skills

• Active, unrestricted licensure and relevant certification in primary care
• Valid driver's license and reliable transportation
• Ability to navigate varied home environments and perform physical assessments
• Preferred: 1+ years of clinical experience, proficiency with electronic health records, and strong communication skills with diverse populations
