About the role
Join our team as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) providing in-home health assessments across Missouri.
Responsibilities
- Conduct comprehensive evaluations, including medical history, physical exams, screenings, and addressing social determinants of health
- Identify diagnoses and communicate findings to primary care providers
- Educate members on disease management and medications
Schedule
Flexible scheduling without on-call, weekends, or holidays. Travel expenses covered.
Pay

Qualifications
- Active, unrestricted licensure in Missouri
- 1+ years of clinical experience, preferably in family, geriatric, or home health
- Valid driver's license and reliable transportation
- Ability to navigate varied home environments and perform assessments
Preferred Skills
- Proficiency with electronic medical records
- Effective communication with Medicare and geriatric populations
