Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for in-home assessments across Arkansas. This field-based role involves conducting comprehensive health evaluations, including medical history, physical exams, screenings, and medication reconciliation, to improve patient outcomes and facilitate care coordination with primary providers.

Key responsibilities

• Performing in-home assessments and documenting diagnoses
• Communicating findings to primary care providers
• Identifying urgent situations and providing member education
• Addressing social determinants of health and making referrals

Qualifications

• Active licensure and certification, with the ability to practice independently if permitted by state laws
• Reliable transportation and ability to navigate varied home environments
• Preferred: 1+ years clinical experience, proficiency with EMRs, and strong communication skills with Medicare populations

Compensation

Compensation ranges from $109,500 to $164,000, including base pay and bonuses, with comprehensive benefits and incentives.

Schedule

Flexible scheduling, no on-call, weekends, or holidays, and travel expenses are covered.
