Conducts in-home health assessments for Medicare and other plan members, focusing on preventive care, education, and care coordination. The role is field-based, emphasizing holistic, patient-centered care in a collaborative environment.

Responsibilities

• Perform comprehensive assessments
• Document diagnoses
• Communicate with primary care providers
• Recognize urgent situations
• Educate members on health management
• Address social determinants of health

Qualifications & Skills

• Active licensure (NP or PA)
• Certification (ANCC, AANP, NCCPA)
• Reliable transportation
• Ability to navigate home environments
• Preferred experience includes clinical work with geriatric or Medicare populations
• Preferred proficiency with electronic medical records

Schedule

• Flexible scheduling (8am–8pm)
• No on-call, weekends, or holidays
