Traveling Nurse or Physician Assistant - House Calls

About the role

We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments across Washington state. This role focuses on comprehensive health evaluations, addressing social determinants of health, and providing patient education and referrals as needed.

Responsibilities

• Conduct comprehensive in-home health evaluations
• Document diagnoses and communicate findings with primary care providers
• Recognize and address urgent health issues
• Educate patients on health management and preventive care
• Address social determinants of health and provide appropriate referrals
• Perform assessments in varied home environments

Requirements

• Active, unrestricted licensure in Washington state
• For Nurse Practitioners: Certification from ANCC or AANP
• For Physician Assistants: NCCPA certification and licensure
• Reliable transportation and ability to navigate home environments
• Experience with electronic medical records preferred

Schedule
