Nurse Practitioner or Physician Assistant House Calls - Traveler

About the Role

We are seeking a Nurse Practitioner or Physician Assistant to conduct comprehensive in-home health assessments for Medicare Advantage members. The role focuses on preventive care, patient education, and closing care gaps by evaluating medical history, medications, vital signs, and performing screenings. Findings will be communicated to primary care providers to improve health outcomes, address social determinants of health, and identify urgent health issues.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage members.
• Evaluate medical history, medications, and vital signs.

Requirements

• Active licensure and certification (ANCC/AANP for Nurse Practitioners or NCCPA for Physician Assistants).
• Unrestricted driver’s license and reliable transportation.
• Ability to navigate varied home environments and perform physical assessments.

Preferred Qualifications

• Clinical experience, particularly with older populations.
• Proficiency with Electronic Medical Records (EMRs).
• Strong communication skills.
