Traveling Nurse or Physician Assistant - House Calls

About the Role

This position involves conducting in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care coordination. The role requires extensive travel across the state, with all expenses covered, and offers flexible scheduling without on-call, weekends, or holidays.

Responsibilities

• Perform comprehensive assessments including medical history, vital signs, and screenings
• Identify diagnoses, document findings, and communicate with primary care providers
• Recognize urgent situations and provide appropriate interventions
• Educate patients on health conditions, medications, and social needs
• Address social determinants of health and facilitate referrals

Requirements

• Active licensure as a Nurse Practitioner (ANCC/AANP) or Physician Assistant (NCCPA)
• Unrestricted license in the practicing state and valid driver’s license
• Ability to navigate diverse home environments and perform assessments physically
