Nurse Practitioner - In-home Visits

We are seeking board-certified nurse practitioner contractors to perform in-home health evaluations for members covered by Medicare Advantage, Medicaid, and certain commercial plans. Responsibilities include conducting comprehensive health and well-being assessments, reviewing medications and medical history, and performing physical evaluations in patients' homes, without prescribing medications or modifying treatments. Key duties involve pre-scheduled visits, identifying care barriers, and providing patient education to empower health management. Qualifications include state licensure and willingness to enroll in Medicaid if needed. Skills required are strong communication, clinical assessment, and flexibility.
