Nurse Practitioner - In-home Visits

About the role

We are seeking board-certified nurse practitioner contractors to perform in-home health evaluations for members covered by Medicare Advantage, Medicaid, and certain commercial plans.

Responsibilities

• Conducting comprehensive health and well-being assessments
• Reviewing medications and medical history
• Performing physical evaluations in patients' homes, without prescribing medications or modifying treatments
• Pre-scheduled visits
• Identifying care barriers
• Providing patient education to empower health management
