About the role
Board-certified nurse practitioner contractors conducting in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members.
Responsibilities
- Perform in-home visits to assess overall health and identify care barriers
- Review medication and medical history
- Conduct physical evaluations
- Manage pre-scheduled visits
- Provide assessments without prescribing medication
- Support member education
Compensation & Benefits
- Earn up to $2,500-$4,000 weekly, with potential for increased rates up to 135% through Market Multiplier and pay-per-test incentives

Qualifications & Skills
- State license required
- Medicaid license or willingness to enroll preferred
- Independent contractor (1099), with the ability to manage a flexible schedule
