About the role
Board-certified nurse practitioner contractors conduct in-home health evaluations for healthcare plan members.
Responsibilities
- Perform comprehensive in-home assessments focusing on overall health, medication and medical history review, and physical evaluations without prescribing medication or ordering tests.
- Identify potential care barriers and provide education to promote health management.
- Complete pre-scheduled in-home visits, typically 6-9 daily.
Requirements
- State licensure required; Medicaid license or willingness to enroll preferred.
- Ability to work independently with a flexible schedule.
- Strong communication skills and commitment to patient-centered care.
Pay
Earn up to $2,500–$4,000 weekly, with potential to increase via market multipliers and pay-per-test models.
Schedule
- No nights or weekends required.
- Shifts available across all days.
