About the role
This role involves conducting in-home health assessments for Medicare Advantage and other plan members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers.
Responsibilities
- Performing comprehensive assessments, including medical history, medication reconciliation, vital signs, and physical exams
- Identifying and documenting diagnoses for care management and treatment planning
- Communicating findings to primary care providers (PCPs) to address care gaps
- Recognizing urgent situations and intervening appropriately
- Educating members about disease management, medications, and compliance
- Addressing social determinants of health and providing referrals as needed
Qualifications
- Active licensure and certification as an NP or PA
- Unrestricted state licensure and valid driver’s license
- Ability to navigate varied home environments and perform assessments requiring sitting, standing, or kneeling
- Preferred experience in clinical settings
- Proficiency with electronic records
- Strong communication skills with older or Medicare populations
Pay
Salary ranges from $109,500 to $164,000, including base pay, bonuses, and incentives.
