NP/PA - House Calls Traveler

About the role

This role involves conducting in-home health assessments for Medicare Advantage and other plan members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers.

Responsibilities

• Performing comprehensive assessments, including medical history, medication reconciliation, vital signs, and physical exams
• Identifying and documenting diagnoses for care management and treatment planning
• Communicating findings to primary care providers (PCPs) to address care gaps
• Recognizing urgent situations and intervening appropriately
• Educating members about disease management, medications, and compliance
• Addressing social determinants of health and providing referrals as needed

Qualifications

• Active licensure and certification as an NP or PA
• Unrestricted state licensure and valid driver’s license
• Ability to navigate varied home environments and perform assessments requiring sitting, standing, or kneeling
• Preferred experience in clinical settings
• Proficiency with electronic records
• Strong communication skills with older or Medicare populations

Pay

Salary ranges from $109,500 to $164,000, including base pay, bonuses, and incentives.
