Healthcare Provider - House Calls (NP or PA)

About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on improving health outcomes through education, identifying care gaps, and collaborating with primary care providers.

You will review medical history, conduct physical exams, perform screenings, recognize urgent situations, and address social determinants of health. Flexible scheduling options are available, with no on-call, weekends, or holidays required.

Responsibilities

• Perform in-home health assessments for Medicare Advantage and other plan members
• Review medical history and conduct physical exams
• Perform health screenings and identify care gaps
• Recognize and address urgent health situations
• Collaborate with primary care providers to improve health outcomes
• Educate members on health-related topics
• Address social determinants of health

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, or NCCPA)
• Reliable transportation and ability to navigate home environments

Qualifications

• Preferred: 1+ years of clinical experience
• Proficiency with EMRs
• Strong communication skills, particularly with geriatric/Medicare populations
