Healthcare Provider - House Calls

About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage and other plan members. Your responsibilities include reviewing medical histories, conducting physical exams, identifying diagnoses, and communicating findings to primary care providers to bridge care gaps. You will educate members on health conditions, medications, and social determinants, intervening in urgent situations as needed. This non-prescribing, field-based role emphasizes improving health outcomes through preventive care and collaboration in an interdisciplinary environment.

Requirements

• Active, unrestricted licensure and certification (ANCC, AANP, NCCPA)
• Valid driver’s license and reliable transportation
• Ability to navigate varied home environments and perform assessments

Preferred Qualifications

• 1+ years of clinical experience
• Proficiency with electronic medical records
• Strong communication skills with geriatric or Medicare populations
