Healthcare Provider - House Calls (NP or PA)

About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care to improve health outcomes. You will educate patients on disease management, address social determinants of health, and recognize urgent situations requiring intervention.

Responsibilities

• Conduct physical exams and review medical histories
• Perform medication reconciliation
• Identify diagnoses and communicate findings to primary care providers
• Navigate home environments to perform assessments

Requirements

• Active, unrestricted licensure and certification (ANCC/AANP for NPs, NCCPA for PAs)
• Valid driver’s license and reliable transportation
• Ability to perform physical assessments in home environments

Qualifications

• Preferred: 1+ years of clinical experience
• Proficiency with electronic health records
• Strong communication skills with geriatric or Medicare populations
