About the Role

The Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts in-home health assessments for Medicare members, focusing on preventive care, education, and care gap closure. The role involves reviewing medical histories, performing physical exams, documenting diagnoses, and collaborating with primary care providers to improve health outcomes.

Responsibilities

• Conduct comprehensive assessments, screenings, and point-of-care tests
• Communicate findings to primary care providers
• Identify urgent situations and intervene appropriately
• Educate members on health conditions and medications
• Address social determinants of health and provide referrals

Requirements

• Active, unrestricted state licensure and applicable certifications
• Reliable transportation and ability to navigate varied home environments
• Proficiency with electronic medical records

Preferred Qualifications

• 1+ years clinical experience
• Excellent communication skills
• Ability to transport equipment

Pay

Salary ranges from $109,500 to $164,000 plus bonuses.
