Nurse Practitioner, House Calls

About the role

We are seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on improving health outcomes through education, identifying care gaps, and collaborating with primary care providers.

Responsibilities

• Comprehensive in-home health assessments
• Documenting diagnoses and findings
• Communicating findings to primary care providers
• Recognizing and responding to urgent situations
• Educating members on health conditions and care options
• Addressing social determinants of health

Qualifications

• Active certification in family, adult, geriatric, or adult-gerontology primary care
• Valid licensure as a Nurse Practitioner or Physician Assistant
• Reliable transportation
• Ability to navigate home environments safely

Preferred experience

• Clinical practice in family, geriatric, or home health settings
• Proficiency with electronic health records
• Strong communication and interpersonal skills

Pay

Compensation ranges from $109,500 to $164,000.
