About the role
Join a team providing in-home, preventive healthcare assessments for Medicare Advantage and other plan members.
Responsibilities
- Conduct comprehensive assessments: medical history, medication reconciliation, vital signs, physical exams
- Identify and document diagnoses for care management
- Collaborate with primary care providers to address care gaps
- Recognize urgent situations and intervene appropriately
- Educate members on health conditions, medications, and compliance
- Address social determinants of health and make referrals
Requirements
- Active licensure (NP or PA) and certification in relevant specialties
- Unrestricted driver’s license and reliable transportation
- Ability to navigate diverse home environments and perform assessments
- Eligibility to practice independently if applicable
Preferred Skills
- Clinical experience in family, geriatric, or home health care
- Proficiency with electronic medical records
- Strong communication skills with geriatric or Medicare populations
