About the role
This role involves conducting comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and care coordination.
Responsibilities
- Performing medical history reviews, physical exams, screenings, and point-of-care testing
- Documenting diagnoses and communicating findings with primary care providers
- Identifying urgent situations and providing patient education on diseases and medications
- Addressing social determinants of health and making referrals as needed
Qualifications
- Active licensure and certifications (ANCC, AANP, NCCPA), with ability to practice independently if applicable
- Valid driver's license and reliable transportation
- Bilingual in Spanish preferred
- Ability to navigate home environments and perform assessments physically
Preferred Skills
- 1+ years of clinical experience
- Proficiency with electronic medical records
- Strong communication skills with geriatric or Medicare populations
