Nurse Practitioner or Physician Assistant - House Calls

About the role

This role involves conducting comprehensive in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and care coordination.

Responsibilities

• Performing medical history reviews, physical exams, screenings, and point-of-care testing
• Documenting diagnoses and communicating findings with primary care providers
• Identifying urgent situations and providing patient education on diseases and medications
• Addressing social determinants of health and making referrals as needed

Qualifications

• Active licensure and certifications (ANCC, AANP, NCCPA), with ability to practice independently if applicable
• Valid driver's license and reliable transportation
• Bilingual in Spanish preferred
• Ability to navigate home environments and perform assessments physically

Preferred Skills

• 1+ years of clinical experience
• Proficiency with electronic medical records
• Strong communication skills with geriatric or Medicare populations
