About the Role
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct comprehensive in-home health assessments for Medicare Advantage and other plan members. The role focuses on preventive care, patient education, and closing care gaps through detailed medical reviews, physical exams, and collaboration with primary care providers.
Responsibilities
- Review medical history and perform physical examinations in patients' homes.
- Identify diagnoses and communicate findings to primary care providers.
- Recognize urgent medical situations and take appropriate action.
- Address social determinants of health through referrals and resources.
- Provide patient education to improve health outcomes.
- Collaborate with care teams in a flexible, travel-intensive environment.
Requirements
- Active, unrestricted licensure in your state, with ability to obtain or practice independently if applicable.
- For Nurse Practitioners: Certification from ANCC or AANP in family, adult, or geriatric primary care.
- For Physician Assistants: NCCPA certification and state licensure.
- Valid driver’s license, reliable transportation, and ability to navigate varied home environments.
Preferred Qualifications
- 1+ years of clinical experience.
- Proficiency with electronic medical records (EMRs).
- Strong communication skills and ability to handle medical equipment during home visits.
