About the role

Join a patient-centered home care program as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).

Responsibilities

• Conducting comprehensive in-home health assessments, including medical history, physical exams, and screenings
• Identifying diagnoses, documenting findings, and communicating with primary care providers
• Recognizing urgent situations and providing appropriate interventions
• Educating patients on disease management, medications, and addressing social determinants of health

Qualifications

• Active certification (ANCC, AANP, NCCPA) and unrestricted licensure in the state of practice
• Valid driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments in different physical settings

Preferred qualifications

• 1+ years of clinical experience
• Proficiency with electronic health records
• Strong communication skills with geriatric or Medicare populations
