Nurse Practitioner or Physician Assistant - House Calls

Responsibilities

• Conduct comprehensive in-home health assessments, including review of medical history, medications, vital signs, and physical exams.
• Identify diagnoses and document findings for care management.
• Communicate with primary care providers regarding patient care.
• Recognize and respond to urgent medical situations.
• Educate members on health topics and preventive care.
• Address social determinants of health and provide referrals as needed.

Qualifications

• Active certification (ANCC, AANP, or NCCPA).
• Unrestricted licensure as a Nurse Practitioner or Physician Assistant.
• Valid driver’s license and reliable transportation.
• Ability to navigate and perform assessments in varied home environments.

Preferred qualifications:

• 1+ years of clinical experience.
• Proficiency with electronic health records.
• Strong communication skills, particularly with Medicare populations.
