Nurse Practitioner, House Calls

About the Role

As a HouseCalls Advanced Practice Clinician, you will perform comprehensive in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care gap closure.

Responsibilities

• Review medical history and conduct physical exams
• Facilitate screenings and document diagnoses
• Communicate findings and recommendations to primary care providers
• Recognize and respond to urgent medical situations
• Address social determinants of health through referrals

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in Missouri or ability to obtain it
• Valid driver’s license and reliable transportation
• Ability to navigate home environments and perform assessments

Preferred Qualifications

• 1+ years of clinical experience (family, geriatric, or home health preferred)
• Proficiency with electronic medical records and technology
• Ability to transport equipment and navigate stairs during home visits
• Strong communication skills with geriatric or Medicare populations
