About the role

Join an innovative in-home healthcare team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant). This role focuses on preventive, patient-centered care, improving health outcomes through education and collaboration.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, physical exams, vital signs, and screenings.
• Identify diagnoses for care management and collaborate with primary care providers to address care gaps.
• Recognize urgent situations and provide appropriate interventions.
• Educate patients on health conditions, medications, and adherence.
• Address social determinants of health and make referrals as needed.

Qualifications

• Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted licensure in Missouri or ability to obtain it.
• Reliable transportation and ability to navigate diverse home environments.

Preferred: 1+ years of clinical experience, proficiency with electronic medical records, and strong communication skills with geriatric or Medicare populations.

Schedule
