Join a innovative in-home healthcare team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant). Responsibilities include: Conducting comprehensive in-home health assessments, including medical history, physical exams, vital signs, and screenings.Identifying diagnoses for care management and collaborating with primary care providers to address care gaps.Recognizing urgent situations and providing appropriate interventions.Educating patients on health conditions, medications, and adherence.Addressing social determinants of health and making referrals as needed. Qualifications: Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.Unrestricted licensure in Missouri or ability to obtain it.Reliable transportation and ability to navigate diverse home environments.Preferred: 1+ years of clinical experience, proficiency with electronic medical records, and strong communication skills with geriatric or Medicare populations.
