About the role
Join a team providing in-home preventive care as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conducting comprehensive in-home health assessments, including medical history, physical exams, and screenings
- Identifying diagnoses and documenting findings for care management
- Communicating with primary care providers to address care gaps
- Recognizing urgent situations and providing appropriate interventions
- Educating patients on health conditions, medications, and adherence
- Addressing social determinants of health and making referrals
Requirements
- Active licensure and certification (ANCC, AANP, NCCPA) in relevant fields
- Unrestricted driver's license and access to reliable transportation
- Ability to navigate varied home environments and perform assessments
- State-specific practice authorization if required
Preferred Skills
- 1+ years of clinical experience, especially in geriatrics or home health
- Proficiency with electronic medical records and technology
- Ability to handle equipment and navigate stairs during home visits
- Strong communication skills with Medicare and geriatric populations
