About the role

Seeking a Field-based Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments across North Carolina.

This role is dedicated to providing preventive, holistic care to improve patient outcomes and address health disparities, emphasizing equitable and accessible healthcare.

Responsibilities

• Conducting comprehensive assessments, including medical history, physical exams, screenings, and point-of-care testing
• Identifying diagnoses for care management and communicating findings to primary care providers
• Recognizing urgent situations and providing member education on health conditions and medications
• Addressing social determinants of health and making appropriate referrals
• Ensuring quality care while working independently in members' homes

Requirements

• Active licensure (NP or PA) in the state of practice with relevant certifications
• Valid driver's license and reliable transportation
• Ability to navigate diverse home environments and perform assessments physically

Qualifications

Preferred: 1+ years clinical experience, proficiency with EMRs, and strong communication skills with Medicare populations.

Pay

Salary ranges from $91,700 to $163,700 annually, plus incentives.
