About the role
We are seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare and other plan members across the state.
Responsibilities
- Perform comprehensive assessments including medical history, physical exams, screenings, and point-of-care tests
- Identify diagnoses and collaborate with primary care providers to address care gaps
- Recognize urgent situations and provide member education on health management
- Address social determinants of health and make appropriate referrals
Requirements
- Active licensure and certification as a Nurse Practitioner or Physician Assistant
- Reliable transportation and ability to navigate diverse home environments
Qualifications
Preferred experience includes:
- Clinical practice with geriatric or Medicare populations
- Proficiency with electronic health records
- Strong communication skills
