About the role
Join a patient-centered home visiting team as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conducting comprehensive in-home health assessments, including medical history, physical exams, screenings, and point-of-care testing
- Documenting diagnoses for care management and collaborating with primary care providers
- Identifying urgent situations, educating members on health topics, and addressing social determinants of health
- Providing referrals and ensuring quality care to improve health outcomes
Requirements
- Active licensure and certification relevant to the role, with ability to practice independently if applicable
- Reliable transportation and ability to navigate varied home environments
Qualifications

Pay
Competitive salary based on experience, ranging from $91,700 to $163,700 annually, plus incentives.
