About the role
Join a patient-centered home care team as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, vital signs, and physical exams.
- Identify diagnoses and communicate findings to primary care providers.
- Recognize urgent situations and take appropriate action.
- Educate members on health management and self-care.
- Address social determinants of health through referrals and community resources.
Requirements
- Active certification (ANCC, AANP, or NCCPA).
- Unrestricted state licensure.
- Valid driver’s license and reliable transportation for home visits.
- Ability to navigate varied home environments and perform physical assessments.
Preferred Skills
- 1+ years of clinical experience, especially in family, geriatric, or home health care.
- Proficiency with electronic medical records and technology.
- Strong communication skills, particularly with Medicare or geriatric populations.
Schedule
- Flexible scheduling.
- Approximately 85% travel across North Carolina and South Carolina (all expenses covered).
- No weekends or holidays required.
