About the Role
The HouseCalls Advanced Practice Clinician conducts in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care, education, and care gap closure. This non-prescribing, field-based position emphasizes collaboration with primary care providers to improve health outcomes while addressing social determinants of health.
Responsibilities
- Perform comprehensive assessments including medical history, vitals, and physical exams
- Identify diagnoses and document findings for care management
- Communicate with primary care providers to coordinate care
- Recognize urgent situations and intervene appropriately
- Educate members on health, medications, and adherence
- Address social factors and provide referrals as needed
Requirements
- Active, unrestricted licensure and certification (NP or PA) in relevant fields
- Reliable transportation and valid driver's license
- Ability to navigate home environments and perform assessments in varied settings
Preferred Qualifications
- 1+ years clinical experience
- Proficiency with electronic records
- Strong communication skills with geriatric or Medicare populations
